United Pioneer Home’s Long-term Care community provides medical and personal care with a strong emphasis on safety, dignity, and comfort. We focus on person-centered choices within two neighborhood-style settings, ensuring our caregivers support our residents’ physical health and human spirit.

Our commitment to a person-centered model of care empowers residents to make choices and decisions, respecting their individual preferences. We prioritize building strong relationships among residents, caregivers, families, and volunteers through continuous interaction and support.

Each neighborhood at United Pioneer Home features welcoming environments with shared amenities, including kitchens, dining areas, and cozy fireplaces. 

In each neighborhood, residents will find a:

Our private and semi-private rooms are designed to be bright and cheerful, providing a welcoming atmosphere for residents. Each room has a private bathroom featuring a shower, ensuring comfort and convenience. We believe that a positive living space contributes to the overall well-being of our residents, promoting a sense of home and comfort.

Residents enjoy made-to-order breakfasts from the neighborhood kitchen, while the main kitchen at the care center provides other meals with a variety of options and portion sizes.

The neighborhood-style settings foster better connections among residents, their families, and staff, promoting the development of meaningful relationships.

Long-term care residents can also access all the United Pioneer Home campus amenities. 

For long-term care, we accept Medical Assistance, long-term care insurance plans, and private and personal funds. This flexibility ensures that we can cater to the needs of our residents and their families, providing the necessary support for their care.

Transitional Care Services

Transitional care is a supportive environment for healing between hospital discharge and returning home after an injury or illness. 

Our transitional care program has specialized in rehabilitative care for individuals following their hospital stay. With a focus on collaboration, our experienced team works closely with you and your loved ones to develop a personalized care plan that begins on your first day and guides you toward a successful return home.

Patients need to meet specific admission criteria. Once their condition improves, they will be discharged either back to their homes or to a more suitable placement. In certain cases, there is also the option for long-term stay on our campus.

We collaborate with Greenfield Rehabilitation to offer onsite physical, occupational, and speech therapies. This partnership enhances our transitional care program, ensuring that our patients receive comprehensive and specialized therapy services tailored to their recovery needs while staying with us.

Payment for transitional care is typically provided through Medicare, Managed Care, or Private Insurance. This financial support helps ensure that patients receive the necessary rehabilitative services during their recovery period.
